Entrance Health Requirements for Non-Healthcare Students

Requirements for non-healthcare undergraduate, graduate, continuing studies, non-degree, or professional program:

• Print and complete the Admission Health Record for Non-Healthcare Students (PDF). Instructions for completion are listed on form.
• Have your healthcare provider complete "PART II: Required Immunizations" in the Admission Health Record or submit copies of previous immunization records per the instructions.
• Review the required immunizations for non-healthcare students which is also listed on the Admission Health Record.

Submit the Admission Health Record and proof of immunizations to:
